Request for Degree Progress Update

Coastline’s military evaluation department will provide students with a degree progress report once per year and if the following conditions exist. Please complete the form below to request a copy of your SOC Agreement or an update of your degree progress.

	1. I have an Official SOC Agreement with Coastline. - AND -
	2a. Prior course work which I took in high school or at other colleges was not reflected in my original SOC Agreement. - OR - Institutional 1: Date Transcript submitted to Coastline: Institutional 2: Date Transcript submitted to Coastline: Institutional 3: Date Transcript submitted to Coastline: Institutional 4: Date Transcript submitted to Coastline:

	2b. I have completed 12 units at Coastline AND have taken additional course work or completed additional military training which I would like evaluated toward my degree. 2b1. I have additional military training and experience which has been posted to my military transcripts that may be applicable toward my degree. I understand that if my original SOC Agreement awarded 40 semester hours from military training and experience, CLEP tests, etc., then any additional credit beyond 40 cannot be applied toward my degree. I submitted my updated military transcript to Coastline on: 2b2. My military or college transcript reflects additional credit which I believe is applicable toward my major or toward a major other than the one previously evaluated toward a degree plan at Coastline. I understand that if my original SOC Agreement awarded 40 semester hours from military training and experience, CLEP tests, etc., then any additional credit beyond 40 cannot be applied toward my degree.
