ORDERING YOUR BOOK AND PDA

The PDA and Book Bundle can be purchased in several ways:

1. ORDER AND PURCHASE THROUGH THE GSA.

2. REQUEST A DEVICE AND TEXTBOOK THROUGH CAP:

Those soldiers with a diagnosed brain injury may qualify for a Pharos device and the memory compensation textbook through the Computer/Electronic Accommodations Program (CAP) Office of the Assistant Secretary of Defense (Health Affairs). Visit http://www.tricare.mil/cap/wsm for further information about the CAP process. You will need to complete the following online forms:

• WSM Needs Assessment - According to the CAP Web site, the information provided in the Needs Assessment is used to identify the best assistive technology solution, given the varying degrees and types of disabilities.
• WSM Request Form - According to the CAP Web site, the request form can be completed by any of the following:
  • An Active Duty Service Member
  • A Case Manager referring on behalf of an Active Duty Service Member
  • A Physician/Therapist referring on behalf of an Active Duty Service Member
